Community Autism Family Fun Time

Play, stay and connect with other families with similar experiences, sharing your lived experiences and how we can make a difference for the wellbeing of your autistic (SEND) children and young people. Session has been funded by NHS South West London Integrated Care Board.

Integrated care is about giving people the support they need, joined up across local councils, the NHS, and other partners including social care providers, voluntary and community enterprise sector and charities. The covers the six local authorities Wandsworth, Merton, Croydon, Sutton, Kingston and Richmond.

A2ndvoice CIC has received funding from South West London Integrated Care Board (SWL ICB) to work with children, young people and their families in Merton and Wandsworth to hear their feedback, views, and experiences about one or more of the following:

· Physical activity - what support and services are important to be healthy and maintain a healthy weight.

. Mental wellbeing – what support and services are important to maintain positive mental health and wellbeing.

· Our local environment - what makes our environments healthy and positive for our physical and mental wellbeing in our communities and health and care services.

· The wider determinants of health and the impact on our physical and mental wellbeing, e.g. employment, housing, education, social connections.

· Transition from child and adolescent mental health services (CAMHS) to adult mental health services

You are invited to take part in Community Autism Family Fun Time to understand your feedback, views, and experiences.

Before you decide if you want to take part, please take your time to read this form and ask any questions you may have. Taking part is voluntary, and you can choose to not take part at any point.

If you are under 13, we need you to take this form home and get your parent or guardian to sign it. If you are 13 or over, you can give us that consent.
